Fasting and Sacrifice Log (blank form)
Here are three files for tracking your prayer intentions and the acts of mortification you offer for them. “Tracking” is perhaps not the best word. Rather, “incentivizing” or “inspiring,” etc. is better. This is because we aren’t trying to look back at a past record; the point is to think about what you can surrender NEXT for the intentions on the list.
In one or two sentences, fill in underneath the title why you are doing this. Then, in the left hand column, prioritize you daily prayer intentions. Write the month (January, February, etc.) above the word “intention.” The 31 days correspond to all the days of the month, and each of these boxes has room for tally marks as you give up something for that intention.

Backstory: I work from home, and the kitchen is just too close. In a moment of boredom / stress I stand up from my computer and start raiding the fridge and pantry. Truly I need more discipline, perhaps the virtue of temperance is utterly absent. Well, here is part of my solution: Restrain the impulse to eat something and record it as a tally mark in one of this log’s many vertical boxes for that particular day.
